What insurance do you accept?
We are proud to accept the following insurance providers: Delta Dental, Cigna, Parkview Signature, Anthem, United Healthcare, and MetLife. We encourage patients to contact us with any and all insurance-related questions as details of specific plans vary.
I have a dental emergency, what should I do?
In the event of a dental emergency, we advise giving our offices a call to determine next steps. If outside of office hours, please leave a detailed voicemail describing your issue and a representative will be in touch shortly. If the emergency is severe or life threatening, please call 911.
What do I need for my first visit?
New patients will need to complete new patient paperwork which can be downloaded and pre-completed prior to your visit, if desired. To download new patient paperwork, click here. Paperwork will be provided upon arrival at our offices if you do not pre-complete. New patients should also bring their dental insurance card or proof of insurance, if available.
Do you offer payment plans?
We are happy to offer 3-month payment plans to our patients upon request in our office. Please ask a representative at your next visit about payment plan options, if desired.
What payment options do you accept?
We accept Visa, Mastercard, Discover and American Express, cash or check. You can also pay with third-party financing through CareCredit, allowing you to make more affordable payments over a longer period of time. We invite you to also to learn more about our in-office Membership Club.
Do you offer teeth whitening treatment?
Yes! We offer ZOOM! whitening which is applied in-office for instant, dramatic results. We also offer bleaching trays with bleach syringes, Sheer whitening strips, and professional strength Crest Whitestrips available upon request. Interested in teeth whitening?
